Backflow authority, device, and notice record

Use the current notice and serving authority. One city’s device, tester, schedule, permit, or reporting rule does not apply everywhere.

Do not adjust, bypass, dismantle, or cover an assembly. Pressure symptoms do not authorize adjustment. Fire systems and other specialized connections require the proper professional and authority boundary.

Test, repair, retest, and acceptance record

A test appointment is not the same as an accepted authority record. Keep each step and confirmation.

  • Current notice and device identifier matched
  • Tester or provider qualification verified with the current program
  • Test date, result, gauges / report, and submission route recorded
  • Failed result or repair need documented without homeowner adjustment
  • Repair scope, permit, and inspection needs confirmed
  • Retest completed when required
  • Authority submission receipt or confirmation number saved
  • Final acceptance / resolved-notice status confirmed
